Please find the CSV Upload template attached.

In order to be able to upload the file:

Parameter formatting needs to match the template exactly
Request Date needs to be within the current month for invoicing purposes
The File needs to be submitted as a CSV/ Text file not an Excel File, please make sure the format still corresponds when saving it.
